Peptide injections are being promoted as a shortcut to faster recovery, easier weight loss and healthier aging.
While some have real medical uses, others come with big promises and very little proof.
“Peptide therapy can mean many different things, and not every product comes with the same level of research or oversight,” says Joe Li, DO, with Hartford HealthCare Medical Group.
So, what is peptide therapy used for? And what should you know before trying it?

Your body already makes peptides
Despite the recent buzz, peptides aren’t new.
Your body makes them naturally to help different systems communicate. They’re involved in processes such as appetite, blood sugar, growth and metabolism.
They essentially work as messengers, carrying instructions that tell certain cells how to respond.
“Many peptide medications are designed to copy or influence signals that already exist in the body,” says Dr. Li. “That can make them useful for treating certain conditions, but it doesn’t mean every product marketed as peptide therapy is safe or effective.”
You may find peptide-based products as injections, pills, nasal sprays or skin care treatments. But what they do, how well they work and how closely they’re regulated can vary significantly from one product to another.

Some peptide medications have proven uses
Peptide-based medications have been part of medical care for years.
Insulin, an essential hormone used to treat diabetes, is one familiar example. More recently, GLP-1 medications such as semaglutide and tirzepatide have become increasingly common. These medications work by mimicking or activating peptide signals that occur naturally in the body.
“There are peptide medications that have been studied extensively and approved for clearly defined medical uses,” says Dr. Li. “In those cases, we have guidance around dosing, side effects and which patients may benefit.”
But FDA approval applies to a specific medication and a specific use. It doesn’t mean every injection described as peptide therapy has gone through the same process.
Even an approved treatment can have risks or be inappropriate for certain patients. That’s why it should be prescribed after a provider reviews your health history and treatment goals.

Wellness clinics are selling something different
The peptide therapy advertised online often looks very different from established medical treatments.
Wellness clinics and online sellers promote peptide injections for concerns ranging from slow workout recovery to poor sleep and changes in body composition.
BPC-157, for example, is often marketed for tendon, ligament and muscle injuries. Ipamorelin and CJC-1295 are promoted as ways to increase growth hormone, build muscle or reduce signs of aging.
But these uses haven’t been supported by the same level of human research required for an FDA-approved medication.
“People may assume a product is safe because peptides occur naturally in the body,” says Dr. Li. “But changing one of those signals can affect more than the result someone is hoping to achieve.”
Some clinics also recommend combining several peptides, sometimes called peptide stacking. That can add even more uncertainty because the combination might not have an established dose or safety record.

Many popular peptide injections lack strong evidence
These treatments can sound convincing. If an injection mimics a signal already in the body, it might seem like it could affect healing, growth or metabolism.
But a promising idea isn’t the same as a proven treatment.
For many of the peptides advertised for wellness or recovery, much of the available research has been done in labs or animals. There may be little reliable evidence showing whether the same benefits occur in people.
“We need strong human studies to understand whether a treatment provides a meaningful benefit and which patients might actually benefit,” says Dr. Li. “Without that information, it’s difficult to know whether the product is doing what the marketing claims.”
Testimonials aren’t proof, either. Improvements may come from the injection, another treatment or the body’s natural healing process.
Research involving one peptide also can’t be used to prove that another peptide works. Each product needs to be evaluated on its own.

Unapproved products can come with added risks
The risks of peptide use can be just as unclear as the potential benefits.
When a peptide hasn’t been well studied in people, there may be no recommended dose, treatment length or clear understanding of long-term effects.
There’s also the question of what’s actually in the injection.
Compounded drugs can be appropriate in some cases, but they aren’t FDA-approved and don’t go through the same review process. Poor compounding practices can lead to contamination or incorrect amounts of an active ingredient.
“With any injection, you need to know what’s in the product and whether it was made, handled and stored properly,” says Dr. Li. “You may not have that assurance when you buy from an unregulated website.”
Products labeled “research use only” or “not for human consumption” are another warning sign.
Any injection can also cause irritation, infection or an allergic reaction, with additional risks depending on how the peptide affects the body.

Talk to your doctor before trying peptide therapy
If you’re considering peptide therapy, start by talking to your doctor about what you’re trying to treat.
Fatigue, poor sleep, trouble losing weight or slow recovery can have many causes, and choosing a peptide based on an ad might mean missing the real issue.
“Talk to your doctor about what you’re experiencing before you decide on a specific product,” says Dr. Li. “That gives us a chance to look for the cause and discuss treatments that have evidence behind them.”
Always be sure to ask whether a product is FDA-approved for your condition, what research supports it and how you’ll be monitored for side effects. And be cautious of sellers that don’t require a prescription or claim one injection can treat several unrelated problems.
