Hartford HealthCare at Home recently opened a location in Southbury that offers resources and programs to better provide home care services to older adults in the Tribury (Southbury, Woodbury and Middlebury) area.

The office is at Heritage Village Commercial Center, 519 Heritage Road, Suite 2A5. The services are designed to help seniors age in place safely – in their homes. Offerings include home health services, hospice and palliative care, the Alzheimer’s and Dementia Program, and Behavioral Health Program. Hartford HealthCare Independence at Home also can provide caregivers.

St. John, formerly with the Visiting Nurses Association, now Hartford HealthCare at Home, is well known to Heritage Village residents as the author of the “Ask the Nurse” column that ran for many years in The Villager newsletter.
