Dr. Christina Pornprasert has joined Integrated Care Partners as a population health clinical pharmacist. She is responsible for population health management, the key to accountable care and health reform as demanded by new payment incentives focused on value-based care. She supports the care management team to ensure patients receive appropriate preventive, chronic, and transitional care. She also provides medication therapy management consultations and direct patient care for high-modifiable-risk patients through home and clinic visits. She also serves as an assistant clinical professor at the University of Connecticut School of Pharmacy.

Dr. Pornprasert graduated from the University of Connecticut with a doctor of pharmacy degree, with a business minor, in 2014. She completed her post-doctoral residency training at VA Connecticut (VACT) Healthcare System, followed by specialty training in ambulatory care at The Johns Hopkins Hospital. She collaborated with chief medical residents at the VA to increase the use of documented pharmacy recommendations. She implemented pharmacist-physician collaborations at The Johns Hopkins Outpatient Center Medical Clinic to improve evidence-based prescribing per national guidelines. At ICP, she will continue to strive for care-team integration to optimize patient safety, quality, and efficiency.

Dr. Pornprasert is originally from Glastonbury. Highlights from her time at UConn include competitive dancing on the UConn Tap Team and serving in Urban Service Track, a program that provides primary care to the urban, underserved population. In her spare time, she enjoys baking, discovering new restaurants, meeting Food Network chefs and traveling.
