Catherine Callan CHFN, APRN, has been named director of clinical integration, quality and transformation for Integrated Care Partners.

In this role, she will assist with practice transformation, helping primary care practices to better manage the work necessary to promote population health management. She will develop and measure clinical program effectiveness and performance to improve quality, lower cost and improve customer experience. Catherine will also lead ICP’s exploration of clinical options aimed at helping our patients remain healthy while remaining at home.

Although new to ICP, Catherine is not a new face at HHC and her prior experience managing complex cardiology patients in the outpatient setting uniquely qualifies her for this new role. She has been a driving force behind the establishment of heart failure bridge clinics across the system to provide a smoother and safer transition for patients from the hospital to the post-acute care setting and reduce 30-day readmissions. As part of her work with the heart failure clinics she has led a multidisciplinary group which developed a standardized patient education booklet that is utilized throughout the system.

Catherine earned her bachelor of science degree from Sacred Heart University and worked as a registered nurse at Yale New Haven Hospital, before pursuing a master of science degree in nursing at Quinnipiac University. She will be with ICP three days a week and will continue to see patients on the remaining two days.
